Passport Requirements

ReminderTo speed-up the processing time remember to: a) fill-out application form completely, b) submit all original documents, and c) photocopy all necessary documents, prior to going to the Consulate General. Remember most forms are available online. Click the service you need:


I. What to bring/mail:

  1. Latest passport
  2. Previous passport with the visa
  3. Three (3) identical colored photos 1.77" × 1.37" (3.5 × 4.5 cm) recently taken, front view, with a white background. (No eyeglasses or sleeveless attires please.) Poor quality digital photographs not allowed.
  4. Alien Registration Card (notarized copy for MAILED applications)
  5. Additional requirements for married female individuals who wish to change their names from their maiden names:
    1. Marriage Contract (see guidelines regarding registration of marriage)
    2. Divorce Decree (see guidelines regarding registration of marriage)
    3. One (1) colored photo 1.77" × 1.37" of husband and wife
  6. Fees (Payable in cash or money order)
    1. $50.00 Passport Booklet
    2. $20.00 Amendment fee (for change of name requests) in lieu of Passport Booklet Fee
    3. $25.00 Report of Marriage (for change of name requests)

II. What to fill-out:

  1. Application Form (notarized for MAILED applications)
  2. For married female individuals changing their surnames:
    1. Report of Marriage Form (notarized if MAILED)
    2. Amendment Form in lieu of Application Form (if passport is still valid, notarized if MAILED)
    3. CFO Info sheet (if married to foreigner)

III. What to photocopy:

  1. Alien Registration Card
  2. Data page of the previous passport, i.e. pages bearing:
    1. Photo of passport bearer
    2. Personal information
    3. Last page bearing signature of signing officer.
    4. US Immigration Arrival Stamps
    5. US Visa
  3. Proof of MIDDLE NAME
  4. For married female individuals changing their names:
    1. Marriage Contract (3 sets)
    2. Photocopy of husband's passport (2 sets)

This is in addition to the requirements for new passports below. All those applying for the replacement of lost passports will be interviewed by a Consular Officer. Processing of lost passport is subject to 30 days waiting period.

I. What to bring/mail:

  1. Police report
  2. Affidavit of Loss (describing in detail the circumstances of the loss or theft of the passport.
  3. Any valid photo identification card (driver's license, ARC, etc.)
  4. Marriage certificate and Report of Marriage (for married women). (Note: Marriages contracted abroad must first be reported to the Philippine Embassy/Post for recognition.)
  5. Any two of the following:
    1. Original/certified true copy of non-availability of birth certificate from the National Statistics Office, Manila
    2. Original/certified true copy of the baptismal certificate
    3. Joint Birth Affidavit by two disinterested persons. (additional requirements for those presenting item a. above)
    4. Voter's Affidavit or Voter's ID
    5. School Records (e.g. Form 137, Transcript of records, diploma)
    6. Proof of service from the Philippine government
    7. Government Service Insurance System (GSIS) or Social Security System (SSS) Records
    8. INS Naturalization Certificate
    9. Certified true copy of the birth certificates of any of the following: spouse, children, brothers/sister.
    10. Death Certificate of spouse or parents.
    11. Seaman's Book
    12. Professional Regulatory Commission Identification
    13. National Bureau of Investigation (NBI) Clearance
  6. Fees:
    • $90.00 Passport Booklet
    • $10.00 Fax
    • $25.00 Notarization of the Affidavit of Loss (optional, can be notarized outside).

II. What to fill-out:

  1. Application Form
  2. Affidavit of Loss (must be notarized)
  3. For married female individuals:
    1. Report of Marriage Form (see guidelines below).
    2. CFO Info sheet (if married to foreigner)

III. What to do:

  1. Photocopy all original documents.
  2. Bring all original and photocopies to the Consulate for your interview.
  3. After the interview, get note/reminder from the processor on what additional documents you have to submit and what day you have to return for the processing of your passport application
  4. Pay $10.00 for fax/communications fee and $25 for notarization.
  5. After 30 days waiting period, present note to the processor to access record.
  6. Pay $90.00 fee for passport booklet
  7. Passport will be released the following working day.

I. Female applicants changing to married names:

  1. For a marriage solemnized outside of the Philippines, but within the jurisdiction of the Philippine Consulate General in New York, the following must be submitted:
  2. For a marriage solemnized in the Philippines - Certified true copy of marriage certificate issued or authenticated by the National Statistics Office (NSO) in Manila and Passport Application Form (if passport is expired) or Passport Amendment Form (if passport is valid).

II. Female applicants changing from married names to maiden names:

  1. For widowed applicants - certified true copy and photocopy of death certificate of husband, or court order of presumptive death.
  2. For annulled marriages - certified true copy and photocopy of annotated Marriage Contract or Certificate of Registration and certified true copy of Court Order.
  3. For divorced applicant's (Allowed only in cases where the non-Filipino spouse acted as the plaintiff; and not allowed when both parties are Filipino citizens at the time of the divorce - original copy of Divorce Decree.

  1. Personal Appearance
  2. Original/certified true copy and photocopy of Birth Certificate from the National Statistics Office in Manila, or if not available, substitute the following:
    1. Original/Certified true copy of non-availability of birth certificate from the National Statistics Office (NSO) in Manila.
    2. Original/Certified true copy of baptismal certificate or any other acceptable public documents.
    3. Joint birth affidavit by two disinterested persons.
  3. Newly-born should be registered with the Consulate (FA Form No. 40). Present child's original birth certificate, parent's marriage contract, and original/photocopies of parent's passport. ($25.00 Fee).
  4. Three identical colored photos (1.77" × 1.37"), recently taken, front view with white background. (No eyeglasses or sleeveless attire, no digital photograph yet.)
  5. For seamen, in addition to items 1-4, must submit valid Seaman's Book, certification/letter from the ship captain regarding present employment, copy of work contract and shore pass.

Please enclose a self-addressed thick padded brown envelope with sufficient postage (USPS Express Mail is suggested for ease of tracking). Allow five (5) working days for processing time and issuance. METERED STAMPS and PERSONAL Checks are not accepted. The Philippine Consulate General is not responsible for any delay or loss during transit by mail. Prior to mailing take down the tracking numbers of the en route and return mail.

An extended form marriage certificate/contract looks just like a typical marriage contract/certificate except that it contains specific civil status information regarding number of previous marriage. When requesting a copy of your marriage contract from the City Hall of New York, or New Jersey indicate that you need the extended form marriage contract. Church wedding certificates of marriage will not be accepted.
